Comprehensive Care Coordination Training Programs for OhioRISE Professionals

TRAINING EVENTS ARE SPONSORED by the Ohio Department of Mental Health and Addiction Services and the Ohio Department of Medicaid. These professional development events are provided at no cost for Ohio’s System of Care workforce. For those outside of Ohio’s System of Care who are interested in training, please click here to inquire.

Virtual Training Policies

To ensure a productive and respectful virtual learning environment, please review the following policies:

  1. Registration: Each participant must register individually and use their unique Zoom link to join each training session. A reconfirmation email containing your unique training link has been sent to registered individuals. This link is essential for attendance tracking and CE verification.

  2. Connection: Participants are responsible for ensuring a stable internet connection. Bandwidth issues should be addressed with your agency to secure a reliable connection. Please note that Chromebooks or similar devices may complicate connectivity and hinder access to chat information. Attendance cannot be verified for participants who phone in with audio only (no video), and they will be removed from the training session.

  3. Artificial Intelligence: To maintain a secure and focused learning environment, AI use is prohibited during training sessions. Any AI detected will be removed. Zoom auto-transcripts are available for accessibility.

  4. Engagement: Participants are expected to review pre-training materials, including handouts (if provided), before each session. Timely login and preparedness are essential. Active participation in group discussions and breakout rooms is encouraged. Behaviors should mirror in-person training settings, minimizing multitasking such as phone calls, side conversations, unrelated online activities, or driving. Cameras must be turned on, and participants should be visible throughout the session. If camera or Wi-Fi issues exist, please resolve them before the training day.

  5. Earn CE/Attendance Certificate: To be eligible for Continuing Education (CE) credits or an attendance certificate, participants must attend at least 90% of the training session (breaks are not included in the calculation). Following the training, a Qualtrics link to complete an evaluation survey will be provided. Licensed professionals should note that the license number provided during registration will be uploaded to CE Broker for easy access to confirmation. Attendance certificates will be emailed within approximately 4 weeks. Please check your Spam/Junk folder before inquiring about certificate status.

January-June 2025 OhioRISE Care Coordination Training Schedule

The following outlines the required training curriculum for OhioRISE Care Coordinators, effective January to June 2025. The training format, session cadence, length, and types have been updated to better align with the current Care Coordinator ecosystem. These changes are a direct response to feedback collected from post-training surveys, CME leadership, technical assistance/consultation, and insights from our content developers and trainers.

Initial Training for Field Readiness

To ensure care coordinators are fully prepared for field readiness, the following initial steps are required:

  • Care Coordinators must complete the knowledge attainment questions associated with the video training and submit them to their supervisor.
  • Care Coordinators and their supervisors are required to complete the field-based readiness demonstration of skills checklist.
  • Upon completion of the video training, knowledge attainment questions, and supervisor checklist:

Within 60 days of hiring, Care Coordinators are required to complete both the Practice Pillars Phase and the Model Specific Phase training listed below to continue practicing in the field.

Care Coordination Practice Pillars Training Phase

Care Coordinators are expected to participate in all of the following 3-hour live virtual training events focusing on the foundational practice pillars of wraparound. These weekly sessions can be taken in any order. Both Intensive and Moderate Care Coordinators must attend one session for each of the universal courses: STRENGTHS, NEEDS, TEAM, and DETERMINED BY FAMILY.

Completion of the asynchronous video training is a prerequisite for these live sessions.

2.75 Continuing Education credits (CEs) are available for Social Work, Counselor, Psychologist, Registered Nurse (RN), Licensed Practical Nurse (LPN), and Chemical Dependency Counselor licenses for ALL Practice Pillar Courses.

Practice Pillar – Strengths: A cornerstone of Wraparound is identifying and leveraging strengths to build effective teams and personalized care plans with families. This session will guide participants in developing and applying strengths inventories within the care planning process. We will explore and practice methods to effectively discover, document, and share strengths. Participants will gain a detailed framework for understanding strengths to enhance their field work.

Practice Pillar – Needs: Identifying and addressing unmet needs is another essential pillar of wraparound practice in care coordination. This session helps participants understand unmet need statements as crucial tools for articulating the underlying conditions that contribute to challenges faced by individuals and families. Participants will learn how to utilize need statements to frame youth and family concerns, focusing on the “why” behind situations. This framework will equip teams to develop effective support plans.

Moderate Care Coordination (MCC): Putting It All Together

Building upon the 4 Wraparound Practice Pillars, this intensive training program delves into the practical implementation of Moderate Care Coordination within OhioRISE. Participants will learn about the 5 core functions of a Moderate Care Coordinator and how to apply these throughout the care continuum with families. Key tools and strategies will be explored and demonstrated, including: conducting a Capacity, Assets, and Resiliency Examination; building a Child and Family Network; Laura Bernstein’s ‘Model for Taking Action’; Reg Revan’s ‘Action Learning Cycle’; and effective transition planning.

High Fidelity Wraparound Facilitation Skills Training

This training program covers essential skills and tasks for facilitating effective High-Fidelity Wraparound (HFWA) meetings. Topics include HFWA meeting format and structure, pre-meeting planning, team formation, and member engagement. Core facilitation skills such as building consensus, establishing shared understanding, expanding options, managing meeting flow, co-creating action plans, and handling challenging interactions will be addressed. Practical microskills including redirecting, reframing, summarizing, validation, and reflecting will be demonstrated.

TROUBLESHOOTING & CARE COORDINATION SUPERVISOR LEARNING COMMUNITY

The Troubleshooting Care Coordination & Wraparound Supervisor Learning Community is designed to support OhioRISE supervisors in applying and implementing practice models within their organizations and teams. Sessions include brief didactic instruction followed by breakout sessions for facilitated troubleshooting and group Q&A. Held on the 3rd Monday of each month, from 9:00-10:00 a.m.

SYSTEM OF CARE CASE APPLIED LEARNING (SoCAL) COMMUNITY

Only for professionals working for OhioRISE as a CME, CME Sub-contractor, or the Aetna Health Plan. Registration is required.

SoCAL is a consultation-based learning community tailored for Care Coordination teams. It aims to bridge the gap between theoretical practice principles and their practical application. Presenting CMEs should register for at least their presentation session, but participation in all sessions is encouraged. Non-presenting CME Care Coordinators and supervisors are welcome to register for any session. Meetings are held from 9:30-11:00 a.m.
