What is the CNR Adult Day Health Care Program?

The Garden of Life program is an innovative and convenient solution designed to provide daytime clinical and social services to adults. It serves as a valuable alternative to traditional full-time residency, allowing individuals to receive professional care while continuing to live at home and maintain their independence. Suspended temporarily due to the COVID-19 pandemic, the reopening of the CNR Adult Day Health Care program marks a significant step in restoring and expanding crucial community services.

Shari Hutchison, Director of Long-Term Services at the CNR, expressed her enthusiasm for the reopening, stating, “We’re thrilled to announce the reopening of the Garden of Life and excited to continue to expand our services in a meaningful way. The Garden of Life will provide access to state-of-the-art services and hopefully instill confidence and comfort for families knowing that their loved ones will receive the highest levels of care.”

Comprehensive Services Tailored to Individual Needs

The Garden of Life is equipped to cater to a diverse range of needs, offering both educational and rehabilitative services. The program’s strength lies in its personalized approach. Caretakers work closely with individuals and their families to develop an individualized care plan tailored to meet each participant’s specific requirements. This includes strategies designed to keep disabled or handicapped individuals active and engaged within their own homes and the wider community. Furthermore, the program emphasizes fostering socialization and motivation, crucial elements for overall well-being and a higher quality of life.

The CNR Adult Day Health Care program focuses primarily on five core areas to ensure comprehensive care:

  • Rehabilitation: Supporting recovery and improvement in physical and cognitive functions.
  • Nursing Care: Providing professional medical attention and health monitoring.
  • Social Work: Offering emotional and social support, and connecting individuals with community resources.
  • Activities of Daily Living: Assisting with personal care needs in a dignified and respectful manner.
  • Engaging Activities: Providing stimulating and enjoyable activities to promote social interaction and mental acuity.

Through consistent communication with physicians and families, the Garden of Life team effectively coordinates a wide spectrum of impactful care services. These include, but are not limited to:

  • Dental and audiology services
  • Occupational, speech, and physical therapies
  • Nutritional needs assessment and planning
  • Medication monitoring and coordination
  • Case management and supportive assistance

CNR: A Center of Excellence in Livingston County

David L. LeFeber, Chair of the Livingston County Board of Supervisors, highlighted the importance of the program’s reopening for the community. “With the reopening of the Garden of Life, the Center for Nursing and Rehabilitation has further cemented itself as a center of excellence within our community. The CNR is thriving with a full complement of services, highly skilled staff and award-winning services – all of which ensures that the residents of Livingston County will have access to high quality, reliable healthcare for years to come.”

The reopening of the Adult Day Health Care program is a testament to the CNR’s resilience and commitment to the community. The facility has overcome challenges, including staffing shortages that impacted nursing and rehabilitation centers across New York State. Through innovative solutions, such as developing its internal workforce and securing significant grant funding, the CNR has not only navigated these obstacles but has emerged stronger, ready to serve the community with renewed vigor.

Investing in the Future of Care

A key factor in the CNR’s enhanced capacity is its strategic shift towards building its own internal workforce. By moving away from reliance on external agencies and focusing on recruiting, hiring, and training its staff, the CNR has achieved higher employee retention rates and a more stable and dedicated team. The establishment of a Certified Nurse Training program in late 2022, approved by the New York State Department of Health, has been instrumental in this transition. The program has already graduated nearly three dozen individuals, with 19 securing full-time positions at the CNR, significantly reducing the dependence on agency staffing.

Furthermore, the CNR is committed to modernizing its technology infrastructure to enhance operations and resident experience. Over $653,000 in grant funding has been secured for vital infrastructure improvements, including an upgraded nurse call station and enhanced wireless technology throughout the facility. These upgrades will improve communication between residents and staff, enhance resident safety, and streamline operating efficiencies.

Another significant grant of $95,519 from the United States Department of Agriculture’s Distance Learning and Telemedicine Grant Program will fund the implementation of a Learning Management System. This system will offer customized content, personalized learning paths, and mobile applications for employee training and professional development, ensuring the CNR staff remains highly skilled and up-to-date with best practices in care.
