Empowering Cumberland County Residents Through the Connected Care Program

The Cumberland County Department of Public Health and the Department of Social Services have joined forces to introduce the Connected Care Program, a vital initiative designed to uplift the health and well-being of individuals and families within our community. This collaborative program directly addresses the social determinants of health, recognizing that factors beyond medical care significantly impact overall health outcomes.

The Connected Care Program deploys dedicated teams comprised of case managers, public health educators, and community social services assistants. These teams work directly with Cumberland County residents facing a range of challenges, including health issues, food insecurity, housing instability, unmet social or emotional needs, and unemployment. To ensure comprehensive support, each team benefits from the expert consultation of a nurse and a nutritionist.

The cornerstone of the Connected Care Program is personalized care. Teams provide case management and care coordination services tailored to each individual’s or family’s unique circumstances. These services are delivered in a variety of accessible formats, including in-person meetings at home or in office settings, telephone consultations, and virtual interactions, ensuring convenience and flexibility for participants.
