Understanding the Maryland Primary Care Program (MDPCP)

The State of Maryland is proactively transforming its healthcare system through the Total Cost of Care All-Payer Model, a contract with the Federal Government designed to enhance patient care coordination across hospitals and other healthcare settings. This initiative aims to improve overall health outcomes for Maryland residents while effectively managing the increasing costs of healthcare. A cornerstone of this transformative model is the Maryland Primary Care Program (MDPCP).

The MDPCP is a voluntary program specifically created for Maryland primary care providers who meet certain qualifications. It offers crucial funding and resources to support the delivery of advanced primary care services throughout the state. By participating in the Maryland Primary Care Program (MDPCP), primary care practices are empowered to take on a greater role in preventative care, the effective management of chronic diseases, and ultimately, in reducing avoidable hospitalizations.

The Maryland Department of Health (MDH) has collaborated closely with the Centers for Medicare and Medicaid Services (CMS) to develop and implement the Maryland Primary Care Program (MDPCP). The Program Management Office (PMO) within MDH’s Public Health Services, in partnership with the Center for Medicare and Medicaid Innovation (CMMI), is directly responsible for overseeing the implementation of this innovative healthcare transformation program.

Currently, there are no scheduled application periods for practices, Federally Qualified Health Centers (FQHCs), and Certified Technology Organizations (CTOs) to join the Maryland Primary Care Program (MDPCP).
