Your Dedicated Care Team
Within the Mayo Clinic Complex Care Program, your child will be supported by a dedicated team comprising a pediatric provider and a nurse care coordinator. Working collaboratively with your child’s broader care team, they will help you comprehend, organize, and implement your child’s personalized treatment plan. How to Schedule an Appointment

What to Expect After Enrollment in the Program
Once your child is enrolled in the Mayo Clinic Complex Care Program, you can generally expect to meet with your Complex Care Clinic pediatrician every six months. Additionally, you will have either a scheduled visit or phone consultation with your nurse coordinator every three months. Should your child’s condition change or if you return to Mayo Clinic for other appointments, your Complex Care Team will arrange follow-up care sooner, as needed, to ensure continuous support.
Your Complex Care Team will proactively communicate with your child’s local primary care provider, maintaining contact via phone and providing regular updates on your child’s progress and treatment plan.
